Cost of the Immigration Physical

Our fee is based on age:


  • Children less than 2 years old: $300
  • 2 to 17 years old: $325
  • 18 to 24 years old: $350
  • 25 years and above: $325


This all-inclusive fee covers:


  • Physical examination
  • Required lab testing
  • Form I-693 completion and sealing
  • Vaccine review and immunization coordination
  • One sealed USCIS I-693 form + one personal copy


Please note: Vaccinations are not included in the fee and may require additional cost. Immigration physicals are not covered by insurance. We accept cash and all major debit and credit cards.

    A: Immigration exams are not covered by health or travel insurance. 

  • Q: What if I don’t have all the required vaccines?

    A: You can receive the required vaccines at our clinic for an additional fee, or you may choose to get them from a local pharmacy or your local health department.

  • Q: What forms of payment are accepted?

    A: We accept cash and all major credit and debit cards.

  • Q: What documents should I bring to my appointment?

    A: Please bring:


    • A government-issued photo ID (passport or driver's license)
    • Vaccination records (if available)
    • Any relevant medical records if you have ongoing conditions
  • Q: How long does the exam take?

    A: Most exams take about 30 minutes, depending on complexity.

  • Q: Can I eat or drink before my exam?

    A: Yes, you may eat and drink normally before your appointment.

  • Q: Can everything be completed in one visit?

    A: In most cases, yes. Your physical exam is completed during your visit, and we will provide lab orders for you to complete testing at a pre-arranged laboratory.

  • Q: What lab tests are required?

    A: For patients age 15 and older: Tuberculosis (blood test – Quantiferon), Syphilis, Gonorrhea.

  • Q: What about children under 15?

    A: Children typically require only the TB blood test. Syphilis and gonorrhea testing are not required under age 15.

  • Q: Do you use the TB blood test or skin test?

    A: We use the TB blood test (Quantiferon) for all applicants aged 2 years and older. This method is more accurate and helps avoid false-positive results, particularly in individuals who have received the BCG vaccine.

  • Q: What vaccinations are required by USCIS?

    A: Vaccines required include:


    • MMR (Measles, Mumps, Rubella)
    • Polio
    • Tetanus and Diptheria Toxoids
    • Varicella (Chickenpox)
    • Hepatitis A & B
    • Influenza (seasonal)
    • Haemophilus Influenzae type B (children)
    • Rotavirus (children)
    • Meningococcal and Pneumococcal (for those 65+)
    • COVID-19 (age appropriate)
  • Q: What if I don’t have my vaccine records?

    A: We can arrange appropriate lab tests to confirm immunity or coordinate administration of missing vaccines.

  • Q: Can I still have the exam without my vaccination records?

    A: Yes, you may submit them later. Missing vaccines can be administered or immunity can be confirmed through blood tests.

  • Q: Are vaccinations required during pregnancy?

    A: Some vaccines are not recommended during pregnancy. Please consult with your OB/GYN to confirm what’s safe.

  • Q: What happens if a lab test is positive?

    A: You’ll need to complete follow-up care. Options include:


    1. Visiting your local health department for evaluation and treatment

    2. Establishing with us here at Foundation Family Practice for treatment and clearance.

  • Q: Will I need a chest x-ray?

    A: Only if your TB blood test result is positive.

  • Q: Will I get a copy of my completed I-693 form?

    A: Yes. You’ll receive both the sealed USCIS form and a copy for your personal records.

  • Q: How long is the I-693 form valid?

    A: Once signed by the Civil Surgeon, the form must be submitted to USCIS within 60 days. After submission, it’s valid for 2 years.

  • Q: When will I receive my completed and sealed I-693 form?

    A: Usually within 7 to 10 business days, unless additional testing is needed.

  • Q: How will I receive my sealed form and copy?

    A: You can pick it up in person or request that we send it directly to your immigration attorney for a small processing fee.

  • Q: Do you offer interpretation services during the immigration medical exam?

    A: Yes. We provide an in-house Spanish interpreter for Spanish-speaking patients. Our provider also speaks Yoruba and can communicate directly with Yoruba-speaking patients.

  • Q: What if I speak a language other than English, Spanish, or Yoruba?

    A: If you are not fluent in English and do not speak Spanish or Yoruba, you will need to bring your own interpreter to your appointment. The interpreter must be able to accurately interpret medical and personal information during the exam.

  • Q: Can I use a phone translation app instead of bringing an interpreter?

    A: No, translation apps are not allowed. The interpreter is required to sign the I-693 forms during your immigration medical exam. Because of this legal requirement, an in-person interpreter is necessary to ensure accuracy and compliance with USCIS regulations.

  • Q: Can a family member or friend serve as my interpreter?

    A: Yes. A family member or friend may serve as your interpreter as long as they are fluent in both English and your language, and they must be willing and able to sign the I693 forms during your visit.
